Are all the heat controls on one vacuum line? Mine is stuck on defrost and no heat.
Did you check the upper shock absorber bolts?

It would depend on what quality you want, do you want everything taped off? Blueprinted (interior)? Anywhere from $2-4K for a decent paint job.
Find something like this the right size. I'm not there yet, but there has to be some the right size somewhere....

It looks like a Maverick to me
Backwards because the base is for the opposite side mirror. The seat on the mirror is not symmetrical.
